- Established Patient Form
This form needs to be filled out if you are a returning patient who has had a change of address, insurance, phone number(s), email address and/or medical history. This form is also mandatory if you are a returning patient that has not completed an update form in the past three years.
Treatment for Minors
- Consent to Treat Minor Patient Without Parent/Legal Guardian Present (This form is only accepted for established patients.)
This form is needed if a minor patient plans to attend an appointment at Lakeside Dermatology without their parent or guardian present. This form must be filled out in full for the minor patient to be seen and will be filed in the minor patient's chart for future use.
Medical Records Release
Patients may complete this form if they wish to obtain their records from another doctor for the purpose of being seen at Lakeside Dermatology or if they wish to obtain a copy for their personal benefit. This form is also used if Lakeside Dermatology needs to send medical information from our office to another doctor for the purpose of patient care.

Please fill out this form (completely) if you have been seen by one of our providers within the past year. Please give us at least 2 business days to process your request.
